Patient Logistics Coordinator - Full Time, Hybrid, North Iowa

About the position Provide coordination and support for Mercy Patient Logistics teams to facilitate patient admissions, discharges, intra-hospital and inter-facility transfer processes. Assisting the departments to ensure appropriate placement of patients based on diagnosis and physician preference with consideration of bed availability at both acute and rural facilities. Responsible for monitoring census capacity and alerting the leadership team upon trending thresholds in various markets. Responsible for maintaining accurate bed availability information by analyzing information received on the electronic bed board and from clinical staff to coordinate patient assignments and transfers. This role is intended to help keep the flow of the Transfer Center going by having additional support for the team to obtain the information they need that at times can cause delay in the transfer process. Coordinators can help offset the high peak times by taking on the task that can ease the transfer process. This job description covers the most significant duties performed but does not exclude other occasional work assignments not mentioned.

Responsibilities

  • Directs the daily flow of communication and collaboration between all active team members to ensure the highest level of service is being achieved.
  • Leads process improvement initiatives, including but not limited to, workflow development and business strategies.
  • Organizes key metrics into standard leader reporting.
  • Understands the overall strategy for Mercy One Patient Logistics (Mercy Connect Transfer Center and Patient Flow) and various service lines.
  • Develops strategies, in collaboration with the Mercy Transfer Team, to maintain and support team satisfaction.
  • Coordinates and communicates accurate, current/up-to-date census and patient bed availability information
  • Assists with transfer center calls and communication priorities for transfer center team.
  • Supports and maintains flow of incoming calls between the transfer center team.
  • Demonstrates knowledge and skills to log calls and communicate with customers in electronic documentation system
  • Coordinate with referring hospital staff to obtain pertinent patient information (e.g. face sheet, EKG) for determining patient transfer.
  • Communicate with transfer team or referring staff if information is delayed.
  • Coordinate with logistics team in faxing demographic sheets and summaries to the appropriate facility or department.
  • Demonstrates the ability to place electronic bed order for transfer team
  • Utilizes knowledge of current census information and potential inpatient bed needs to project accurate and appropriate placement of incoming patients.
  • Communicate and coordinate high priority patient movement and navigate barrier patient placement.

Requirements

  • Minimum 6 years applicable experience in healthcare setting
  • High school diploma or GED required
  • Knowledge of Medical Terminology required
  • Knowledge of modern office practices
  • Knowledge and efficient use of basic office equipment (telephone, copier, fax, scanner, etc.)
  • Basic knowledge Microsoft Word, Excel, and Google
  • Basic knowledge of databases
  • Strong written and oral communication skills
  • Detail oriented, organized, and ability to prioritize workload
  • Able to handle pressure of time constraints and multi-tasking

Nice-to-haves

  • Associate’s degree or equal experience preferred
