Fully Remote NY/NJ/CT only- Care Coordinator –Chinese/Cantonese/Fuzhounese-Speaking
Asian Community Care Management, Inc. is now hiring a Fully Remote Per Diem / Part-Time Health Home Care Coordinator. You will help clients understand their healthcare benefits, schedule doctor appointments, connect them with community resources (such as housing, food, or mental health services), and check in regularly to make sure they are receiving the care they need. You will also keep simple records and communicate with healthcare providers when needed.

Qualifications:
- New graduates are welcome — no prior experience required
- High school diploma minimum
- Health Home experience is a plus, but not required
Compensation & Bonuses:
- $100 onboarding bonus (complete onboarding within 2 weeks)
- $500–$1,000 sign-on bonus (based on caseload)
- $500 referral bonus (per successful hire)
Compensation
Structure (Monthly | Guaranteed Caseload Available | Up to 50 Cases)
- 10 cases/month — earn approximately $900
- 20 cases/month — earn approximately $2,000
- 21+ cases/month — earn $2,000 + $120 for each additional case
- 50 cases/month — earning potential of up to $5,600/month
- * This is a per-case compensation model (not hourly), offering excellent earning potential based on your availability.
