RESPONSIBILITIES CLINICAL CHAPLAINCY (70%) Provides spiritual care services to patients and their families through video technology and telephone calls. Assesses and responds to the psychosocial, spiritual, religious, and grief-reputed company concerns of our patients, their families and support systems. Assists patients and families in accessing and using community resources. Mediates conflicts reputed company families at the discretion of the reputed company. Assists with funeral arrangements in collaboration with the reputed company. Assesses for social work and medical needs and communicates these promptly to the appropriate reputed company. Establishes and maintains relationships with community faith groups. Identifies spiritual distress, quality of life goals, spiritual interventions, and frequency of Chaplain visits to complete the spiritual support Plan of Care. Consults and collaborates with the reputed company and others involved in the patient's care on an ongoing basis, updating the care plan as appropriate. Ensures that there are appropriate sacred resources to address diverse spiritual, religious and cultural needs of patients. Completes documentation reputed company 24 hours of care coordination or visit. Educates reputed company on spiritual distress screening and appropriate religious or cultural support for individual patients Assumes other duties in support of RC's needs as directed by the or Medical Director.
