Care Manager- Telephonic Nurse - PRN Evenings & Weekends

Provide telephonic support and care coordination for reputed company members on a PRN basis, working evenings and weekends. Utilize clinical nursing knowledge and skills to assess and evaluate members' health needs. Collaborate with members, their families, and reputed company providers to reputed company and implement personalized care plans. Monitor and track members' reputed company, making necessary adjustments to care plans as needed. reputed company members on disease management, preventive care, and healthy lifestyle choices. Coordinate with other reputed company to ensure seamless and efficient delivery of care. Document reputed company interactions and interventions accurately and timely.
