Job Type: Full-time,Remote Responsibilities: Create comprehensive patient assessments & aid in treatment planning via remote settings Complete documentation in compliance with clinical and state requirements Provide excellent, personalized patient care aimed at addressing individual patient concerns See patients for care via telemedicine using the technology platform provided by the practice Makes recommendations to the care teams, effectively communicates, and collaborates with the patient and all disciplines involved in the care of the patients.
