Sr Program Manager, Population Health

About the position The position involves leading and managing the operational and tactical aspects of multiple or enterprise-scale projects focused on population health and health equity. The role requires strategic planning, program management, and collaboration with various stakeholders to ensure successful project implementation and evaluation. Responsibilities • Develop project plans and recommendations to support population health and commercial care management strategy. , • Lead implementation, track, and report on project status. , • Facilitate cross-functional workgroups to define and develop concept designs, business cases, and marketing strategies. , • Develop and track KPIs, budgets, and financial forecasts to support project objectives. , • Generate ideas from inception through program design stages to improve outcomes, focusing on health equity. , • Leverage data and reports to inform interventions and strategies. , • Identify and analyze target audiences and their specific program needs. , • Develop and deploy marketing strategies to increase member engagement in programs. , • Coordinate the development and implementation of policies and procedures for program measurement. , • Control and monitor project effectiveness, updating plans as necessary. , • Ensure compliance with NCQA PHM requirements. , • Represent the organization in meetings with external parties, including vendors and consultants. , • Assist with analysis and development of business processes to ensure quality and cost-effectiveness. , • Develop an escalation process for issues and concerns. , • Create business process improvement initiatives and communication plans. , • Provide indirect reports with clear expectations, feedback, coaching, and training. Requirements • Bachelor's degree and 5 years of experience in project management. , • Master's degree preferred. , • In lieu of degree, 7 years of relevant experience. , • Expertise in public health, health administration, or health care quality.
