Responsibilities

Collect, abstracts and report on data collected from patient medical records. • Identify cases diagnosed and/or treated by referencing primary and secondary sources of information, including the patient’s medical/treatment record, pathology reports, and diagnostic indices. • Abstract reportable registry data and coordinate the long-term follow up data collection and input for registry patients. • Assign medical record codes and enter them into the registry database according to established protocol. • Submit registry data in compliance with the regulatory requirements and timeframes. • Review registry data for completeness and accuracy, and perform follow-up using additional data sources as necessary.

Requirements

Education: • High School diploma equivalency and 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
