Are you an RN looking for to leave the facility rat race but continue in the field you spent years training for? Are you frustrated with not having the time to provide the quality compassionate care you were trained for? Perform routine scheduled visits with clients to monitor care and oversee CNAs and caregivers in the home. Maintain detailed and relevant care plans. Responsibilities  Conduct Weekly and Bi-Weekly visits to Clients  In person home visits as needed within 2hr time period  New clients assessments in a timely manor  Maintain accurate and current care plans  Coordinate Client needs with their PCP as needed  Provide counsel and guidance to family members on managing conditions Requirements:  Active RN License in Virginia or a compact state  Driver's License and vehicle  Current CPR  One year in home care experience preferred Apply Job!Â
