Job Summary

As a Home Health RN Case Manager, you will play a vital role in integrating and developing service plans for our clients, managing their care, and providing guidance to staff. This position involves conducting various visits, including admissions, discharges, recertification, and routine visits. We prefer candidates with Oasis experience, but we are willing to train the right individual.

Key Responsibilities

• Integrate healthcare for clients from pre-admission to post-discharge
• Coordinate and sequence client care to enhance patient flow and interdisciplinary communication
• Evaluate outcomes based on clinical, financial, and utilization data
• Utilize independent nursing judgment and age-specific care methodologies
• Communicate effectively with patients, families, and healthcare teams
• Coordinate, facilitate, and negotiate with others to ensure seamless care

Requirements and Qualifications

• Bachelor's degree in nursing preferred
• Graduate from a nationally accredited nursing program
• Minimum of 1-2 years of professional nursing experience in a clinical care setting
• Currently holds an unencumbered RN license
• Obtains and maintains required department-specific competencies and certifications
