Job Responsibilities:

• Conduct telephonic clinical assessments using approved medical protocols
• Provide recommendations for appropriate levels of clinical care based on clinical judgment and protocols
• Outreach and engage members with programs to promote health and well-being
• Complete clinical eligibility assessments and identify health, condition, or social gaps

Work Schedule:

Monday-Friday: 2 shifts Pacific Time Zone between 8:30 am - 5 pm, then 3 shifts Pacific Time 10:30 am - 7 pm. Quarterly Saturday shifts 8:30 am - 5 pm Pacific time.

Requirements:

• Undergraduate degree or equivalent experience
• Multi-State Compact License
• Outpatient clinical, ICU, ER, PCU, Step-down experience
• Computer proficiency (Microsoft Word, Outlook, and internet)

Preferred Qualifications:

• Call center experience
• Experience with EMR (Electronic Medical Records)
• Recent Case Management experience
• Bilingual - Spanish
