Bilingual RN Patient Care Coordinator
We are looking for a highly organized Registered Nurse (RN) Patient Care Coordinator to join our team. The ideal candidate will be confident and comfortable on the phone, bilingual in English and Spanish, and great at providing support both patients and caregivers.
Key Responsibilities:
- Serve as the primary point of contact for caregivers, providing support, answering questions, and addressing any needs.
- Schedule patients and confirm upcoming appointments with accuracy and professionalism.
- Coordinate care services, ensuring seamless communication between patients, caregivers, and healthcare providers.
- Deliver excellent customer service and maintain a compassionate, patient-centered approach in all interactions.
- Maintain patient records in compliance with company policies and healthcare regulations.
Requirements
- Current licensure as a Registered Nurse (RN) or medical degree.
- Bilingual English and Spanish.
- Strong communication skills, both verbal and written.
- Comfortable managing multiple tasks in a fast-paced environment.
- 1 year of prior experience in patient care coordination, home health, or a similar healthcare setting.
- Proficiency with scheduling systems and basic computer applications.
