Description

This is a full-time Ambulatory Care Clinical Pharmacist position and is fully remote. The hours for this position are Monday-Friday Days.

Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. In this role, the pharmacist will be responsible for conducting comprehensive chart reviews to identify and resolve medication-related issues. Key responsibilities include evaluating potential drug interactions, duplicate therapies, vital signs/laboratory values, and addressing gaps in care related to medication management. The pharmacist performs virtual/phone assessments and drug therapy education. This role will focus on overcoming identified barriers to medication compliance and adherence.

  • Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
  • Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
  • Requires 0-3 years of relevant experience.
  • Requires current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
  • Board certification or other specialized training may be required.
  • Basic Life Support certification through the American Heart Association (AHA) may be required.
  • May require travel to IU Health facilities.
  • May require weekend rotations.
